Fees & Insurance

At the Utah Center for Psychological Services, we offer clear self pay rates and work with a wide range of in network insurance providers. Whether you’re using insurance or paying privately, we believe in upfront costs, no surprises, and guidance you can trust.

  • At this time, our practice is unable to accept new psychological testing/evaluation referrals for BCBS and United members.
If You Would Like to Use Your Insurance:
  • Please verify your mental health benefits prior to your appointment.
  • We will submit claims on your behalf for in network coverage.
  • You are responsible for any copays, coinsurance, or deductibles at the time of service.
Out-of-Network?
If we are not in-network with your plan, we can provide a superbill for you to submit for potential out-of-network reimbursement.

Self Pay Rates

Psychological Evaluations:
  • Psychological Evaluation (e.g., ADHD): $3,100
  • Court-Ordered Psychological Evaluation: $3,100
  • Psychosexual Evaluation: $4,000
  • Violence Risk Assessment: $4,000
  • Parental Fitness Evaluation: $4,000
  • Competency Evaluation: $4,000
  • Personal Injury Evaluation: $400/hour, $4,000 retainer
  • Mitigation & Sentencing Evaluation: $400/hour, $4,000 retainer
  • Criminal Responsibility Evaluation: $400/hour, $4,000 retainer
Therapy Sessions:
  • Initial Intake Session (60 minutes): $300
  • Individual Therapy (50 minutes): $250
  • Family Therapy (50 minutes): $250
Other Psychological Services
  • Expert Testimony: $400/hour, minimum 4-hours
  • Consultation: $400/hour

Payment & Appointment Policies

Find clear information about payment methods, billing, cancellations, and no show policies so you know exactly what to expect before your visit.

For qualifying mental health services, we will bill your insurance if we are in-network. 

If we are not in-network or for services that do not qualify for insurance billing, we will not bill insurance. Instead, these services are subject to our self-pay rates. 

For self-pay evaluations, 50% of the total amount is due to hold the appointment, and the remaining 50% is due on the date of your first appointment.

We accept credit cards and Care Credit.

To avoid a late cancellation fee, please notify us at least 48 hours in advance if you need to cancel or reschedule. Late cancellations or missed appointments may be subject to a $200-$300 fee, which is not billable to insurance.
