Fees & Insurance

At the Utah Center for Psychological Services, we offer clear self pay rates and work with a wide range of in network insurance providers. Whether you’re using insurance or paying privately, we believe in upfront costs, no surprises, and guidance you can trust.

Insurance

We are In Network with the Following Insurance Providers:

  • We do not accept Regence/Blue Cross Blue Shield or United Health Care for psychological evaluations.
If You Would Like to Use Your Insurance:
  • Please verify your mental health benefits prior to your appointment.
  • We will submit claims on your behalf for in network coverage.
  • You are responsible for any copays, coinsurance, or deductibles at the time of service.
Out-of-Network?
If we are not in-network with your plan, we can provide a superbill for you to submit for potential out-of-network reimbursement.

Self Pay Rates

Psychological Evaluations:
  • Psychological Evaluation (e.g., ADHD): $3,100
  • Psychosexual Evaluation: $4,000
  • Violence Risk Assessment: $4,000
  • Parental Fitness Evaluation: $4,000
  • Personal Injury Evaluation: $350/hour, $3,500 retainer
Therapy Sessions:
  • Initial Intake Session (60 minutes): $300
  • Individual Therapy (50 minutes): $200
  • Family Therapy (50 minutes): $200

Payment & Appointment Policies

  • Psychological Evaluation (e.g., ADHD): $3,100

  • Personal Injury Evaluation: $350/hour, $3,500 retainer
To avoid a late cancellation fee, please notify us at least 48 hours in advance if you need to cancel or reschedule. Late cancellations or missed appointments may be subject to a $200-$300 fee, which is not billable to insurance.
